Course Reserve (Submit a ...)

This form allows faculty (or their designees) to request that materials needed by a class be held at a check-out desk in one of the libraries. These materials can then be checked out for limited amounts of time. Please submit one form for each course. This form has space for up to 5 items; please submit additional items on a separate form.

Media Hold

Media Hold requests are used by teaching faculty, staff and graduate teaching assistants who need to use a specific library-owned media item for academic purposes. Once requests are submitted, the item is pulled in advance and placed on the hold shelf at the Ekstrom Library Media Resources Desk.

  • Requests are processed in the order they are received.
  • Media Holds are accepted up to one year in advance.
  • Due to the popularity of certain titles in our collection, titles may not be available on certain dates. Notification of conflict will be sent via email.

Book Holds, Recalls & Searches

Book Hold: Use this option when a book is checked out and you would like the book held for you upon its return.

Book Recall: A request for a library book which has been checked out to be returned as soon as possible. (Only an option for items in Art and Music Libraries. For other libraries, use Interlibrary Loan and indicate in the Notes field that the book is checked out.)

Search: Use this option if the book is not located where Minerva says it is.

Schedule a Lab in Ekstrom Library

Instructions: To schedule a session in one of our instructional labs, 1) check the calendar for conflicts with your time/date, 2) complete the form, 3) read our policies for guidelines and important information, and 4) use one form per class session.

The University Libraries' Reward & Recognition program is designed to identify exemplary performance among libraries faculty and staff.

Any library faculty or staff member may nominate another employee for exhibiting one or more of the distinguishing behaviors. Self nominations can also be made. Deadline for submission is December 5, 2008. Awards will be made on a quarterly basis. The award is a gift card from a local retailer.

Item Holds

A hold requests that an item be kept for you at the check out desk. This is used when: a) a book has been ordered, but is not in the collection yet, or b) when someone else has the book checked out and you would like it held when it is returned.

Authorized (Proxy) User Request

This form allows a patron to designate a specific person to use their library account to check out materials. This form should be completed by the primary patron. **Fields in bold are required. This form requires two business days to be processed.

Order Recommendation

Fill out the form as completely as possible. All fields do not need to be filled in.
